By signing the contract, the Supplier, as a personal data subject, gives their consent to the processing of their personal data (likeness, speech, full name with titles, job title and employer, functions, membership and positions in organizations) presented in the work (lecture) for the purposes of the Course and other educational activities of the LF HK within the framework of the license granted. Data will be reasonably collected, processed, and maintained in accordance with the European GDPR Regulation, Act No. 110/2019 Coll., and other applicable laws, for a period of 5 years from the granting of consent and solely for the stated purpose.

The Supplier has been duly informed of their rights, including the right to withdraw consent at any time without reason, access their data, request rectification, and know that the provision of data is voluntary. They also have the right to know what data are processed, how, and why.

The Supplier acknowledges being informed that if the data processing is considered to conflict with privacy or the law, especially if inaccurate, they may request clarification and correction.

The Supplier is aware of the data protection policies and contacts available on the websites of the Faculty of Medicine in Hradec Králové and Charles University.
